Eligibility

Payment Policy
Administrative fees for letters, forms, and reports are self-pay and are not typically covered by insurance. Payment must be received before work begins.
Once documentation has been prepared, fees are non-refundable.
Prescription and Refill Management
Managing your medications is an important part of your mental health journey. We provide prescription services and medication refills for established patients while monitoring your progress and making adjustments as needed to support your overall well-being.
To request a prescription refill:
- Contact your pharmacy first to determine whether refills are available.
- If no refills remain, ask your pharmacy to send an electronic refill request to our office.
- You may also contact our office if your pharmacy instructs you to do so.
Please allow 2–3 business days for routine prescription refill requests to be reviewed and processed.
Follow-Up Appointments
Our Commitment
Certain medications require regular follow-up appointments before additional refills can be authorized. Keeping your scheduled appointments allows your provider to monitor your progress, evaluate the effectiveness of your medication, and make any necessary adjustments to your treatment plan.
Patients who have not been seen within the recommended follow-up period may be required to schedule an appointment before additional refills can be issued.
