Telehealth Appointment Requirements

To participate in your virtual appointment, please have:

  • Reliable internet connection
  • Camera and microphone
  • Quiet and private location
  • Valid photo identification

Insurance and Payment Information

We accept select insurance plans and self-pay patients.

Please provide your insurance information when scheduling your appointment so we can assist with eligibility verification.

For self-pay rates and payment questions, please contact our office.

Cancellation and Rescheduling Policy

If you are experiencing a medical emergency, having thoughts of harming yourself or others, or require immediate assistance, call 911 or go to the nearest emergency room.

For mental health crisis support, call or text 988, the Suicide & Crisis Lifeline, available 24 hours a day, 7 days a week.

If you need to cancel or reschedule your appointment, please notify our office at least 24 hours in advance.

Late cancellations or missed appointments may be subject to applicable fees.
